If you are a campus employee, you must be enrolled in the UAB Occupational Medicine Program before you are fit-tested or are assigned a respirator for use in your workplace.

Annually thereafter, you must update your medical status and/or be fit-tested as indicated below:

1.	Are you enrolled in the UAB Occupational Medicine Program?	Yes, go to question 2.	No, enroll here.
2.	Have you completed an Initial Respirator Use Form within the last year?	Yes, go to question 3.	No, click here.
3.	Have you been fit-tested within the last year?	Yes, click here.	No, click here.
