The primary purposes of the Faculty Data Form are 

  • to record, track, and manage faculty appointments (primary, secondary, centers, and administrative appointments, volunteers, etc.)
  • to record special appointments, such as Distinguished Professor, University Professor, and Emeritus faculty appointments
  • to enter faculty appointment data into Extra Information Table screens found in Oracle

The Faculty Data Form serves as a primary source of information regarding faculty appointments since the information isn’t captured in Oracle. It’s how secondary and center appointments and faculty tenure status are captured. Records administration enters the data from the paper form into Oracle.
