Instructions for Requesting Proxy Access to a Spouse’s Disclosure Profile
UAB employees are required to disclose applicable financial interests for themselves, spouses, and dependents. To support this requirement among spouses who are both UAB employees, UAB allows employees to request proxy access for a spouse to add their financial interests to an employee’s disclosure profile when both spouses are UAB employees.
Important Requirements
- The spouse who is granting access must submit the request.
- If both spouses want to grant access to one another, each spouse must submit their own request—one request per person granting access.
- Note that spouses who have proxy access may add or change entries in your disclosure profile; however, you (the discloser) must review the changes and submit the disclosure profile update.
How to Request Proxy Access
- Click the link below, ensuring you are logged in using your BlazerID credentials.
- The spouse who is granting access must submit the request.
- If reciprocal access is desired, the other spouse must submit a separate request.
Submit Your Request
Visit the Support Request form for IRAP and myUABResearch
- Select School
- Type of Request = Access, Roles & Permissions
- Enter your Spouse’s BlazerID and ensure the correct name and title displays
- Under “Additional Comments”, include the following: “This individual is my spouse. Please provide proxy access to my disclosure profile.”
Note that each individual is ultimately responsible for their own disclosures of financial interests and must review and submit edits made by their spouse proxy.


Notification of Proxy Access
Once your request is processed, you will receive an e-mail response to the ticket you submitted notifying you that proxy access has been granted to your spouse. At that time, your spouse may log in to myUABResearch and provide disclosure information for you to review and submit, following the step-by-step guide below.

The UAB Enterprise Conflict of Interest and Conflict of Commitment Policy requires certain UAB employees to submit annual disclosures. Individuals with the following roles must fulfill this requirement.
- Investigators. This includes any individual, regardless of title, position, or employment, who is responsible for the design, conduct, or reporting of research, which is defined as any active or pending project processed through OSP or HRPP.
- Full-time faculty. This includes any faculty member with a full-time appointment.
- Institutional Officials. This includes specific employees [i.e., University President, University Provost, University Vice Presidents, Deans, and Chairs] who have direct authority over allocation of institutional resources, assignments of graduate students, trainees, funding, space, salary, or promotions for faculty.
- Other Designated Employees. This includes other administrators in key decision-making, signatory, or assurance roles.

Financial interests include anything of monetary value accepted or owned by an full-time faculty member, investigator, institutional official, his/her spouse, or his/her dependents, not held in an investment vehicle such as a mutual fund or retirement account in which the owner does not directly control investment decisions, whether or not the value is readily ascertainable.
All investigators must complete a Disclosure of Financial Interests prior to undertaking any research, even if there are no financial interests to report.
Investigators must update Disclosure Profiles:
- Within 30 days of acquiring a new interest or experiencing a change in a previously disclosed interest
- After an approved external activity begins
- Annually to re-certify the disclosed interests are still current.
