Forms are allowed on UAB’s websites and can be very useful in collecting information electronically. At UAB, all lead forms, or contact forms that are used primarily for recruitment purposes, should be connected to TargetX, UAB’s content management system. Below are guidelines for using lead forms for undergraduate and graduate recruitment. Undergraduate Lead Forms and Lead Process

  • Schools/department/programs have two options for undergraduate recruitment lead forms on their site:
    • They can link to the existing undergraduate form.
    • They can embed the existing undergraduate form on their website. This is a long form, so it would likely be embedded on a page that is linked to rather than in a footer or sidebar area.
  • Marketing & Communications may use shorter forms for undergraduate leads for the Find Your Degree site and campaigns.
  • Currently, recruiters may not have access to the leads that are collected through the existing undergraduate form. However, enrollment management is working to provide recruiters access to TargetX so they can login and view their leads.

Graduate Lead Forms & Lead Process

  • Graduate programs may use program-specific lead forms on their website.
  • The Graduate School and Marketing & Communications provide services for setting graduate program lead forms, including:
    • Set up of an online lead contact form that is connected to TargetX
    • Development of an initial program overview email that launches when someone completes the form on the website
    • Training on how to log interactions with prospective students and applicants in TargetX

General Contact Forms

  • General contact forms on the Contact Us page and other non-recruitment forms are still allowed on UAB websites. These forms should not be linked to TargetX and should be set up using the approved website form extension. Forms should not be used on the home page of school or department websites. Other Joomla Forms

All forms should abide by UAB’s data security policy which will ensure forms are FERPA and HIPPA compliant.
