Infectious Diseases fellowship applications will only be accepted through ERAS. Please include all of the following information in your application. Missing information will delay review of your application and scheduling of interview:

- CV
- MSPE (Dean’s statement)
- Medical school transcript
- Photo
- ECFMG (if applicable)
- COMLEX transcript (if DO)
- USMLE transcript - Steps 1 & 2
