Your Wellscreen visit will measure blood pressure, total cholesterol, LDL cholesterol, HDL cholesterol, blood sugar, BMI, triglycerides and cholesterol/HDL ratio.

You may have some concerns about the confidentiality about your personal information. In compliance with HIPAA (Health Insurance Portability and Accountability Act), your personal information from a Wellscreen will NOT be shared on an individual level with your employer. Knowing your personal numbers matters. Health screenings can identify unknown conditions, diseases, or risks for disease that can significantly influence the quality of one’s life for the future. A simple drop of blood can provide clues about how to improve one’s health and wellbeing. This is part of UAB’s ongoing commitment to support each employee’s health and wellbeing.

UAB Live HealthSmart Alabama will privately and securely share your Wellscreen data to ICario/Chip Rewards, who will upload your information on your confidential My Health Rewards profile. UAB Live HealthSmart Alabama and ICario/Chip Rewards protect all screening data through secure processes and protocols to ensure confidentiality.

If you are unable to attend one of the screening events, you have the option to have the appropriate measures taken by your healthcare provider using the Wellscreens Physician Fax Form. A Physician Fax Form must be completed by your health care provider and faxed to UAB Live HealthSmart Alabama (205-996-2974) by November 30, 2023, to receive the credit in your My Health Rewards account (if applicable). No exceptions. Please confirm with your physician that your visit is a preventive exam visit to ensure that it is recorded (“coded”) appropriately.

Participants who cannot meet the standards outlined by the Wellscreen program for medical reasons may qualify for a reasonable alternative and may still have an opportunity to qualify for the reward. Please contact UAB Employee Wellness at uabwellness@uab.edu to request a reasonable alternative.
