Individuals (i.e. faculty, staff, students, or members of the community) may communicate concerns to the Department Chair regarding the Doctor of Physical Therapy program and/or behavior of faculty, staff or students by using the form below. Concerns or complaints may be made anonymously or, when possible, the individual completing the form should identify themselves so that action taken may be reported.
Individuals may also contact the Commission on Accreditation of Physical Therapy Education (CAPTE) to file a complaint about the program. Please consult the CAPTE website by clicking on this LINK for filing a complaint with CAPTE.
