Health Insurance
Submit an insurance waiver if you have personal health insurance.

Dental Insurance
Students may be waived from the dental health program if the waiver form and proof of dental insurance is submitted to the Student Dental Health Program. A letter from your dental insurance company with student(s) name, address, social security number and the type of dental coverage provided will serve as proof of insurance.
You may complete the dental waiver in BlazerNET. You will find the waiver under Links/Forms.
