My Records

Employees records may be requested by submitting a medical release form. Upon receiving a request for medical records, your records will be furnish to you within 24-48 hours.

For Hospital employees, requests can be submitted by email to employeehealth@uabmc.edu and/or fax 205-975-6900.

For campus employees, requests can be submitted by email to ehsocchealth@uab.edu and/or fax 205-975-7287.
