All benefits eligible employees and employees who met eligibility for health insurance as defined by the Affordable Care Act (ACA) will receive a 1095-C form at your home address. Do not throw the 1095 form away. Although the 1095 form is not required to file your taxes, it should be kept as part of your tax documentation records.

Truth in Salary

UAB will be providing a Truth in Salary Statement in compliance with a state law to reflect employer contributions to your benefits and salary/wages reported in Fiscal Year 2021-2022. These statements will be available in the UAB for Me Portal by January 31, 2023. A copy will also be mailed to your home address.


Total Compensation Statements

The UAB Benefits Office is pleased to provide an updated Total Compensation Statement from UAB that will be available in March 2023. This statement will be housed in the UAB for Me system and will illustrate the value of your total compensation from UAB, including your salary and benefits. Together, salary and benefit programs represent a significant investment by UAB — designed to recognize your contributions to our collective success.
