1. How do I electronically file a Statement of Economic Interest Form?

2. I was forwarded the ethics requirement email by my coworker/supervisor. Do I need to file?
If you received a direct email from UABCHRO@uab.edu or a letter in the mail addressed to your home, you are required to file the form. If the email was forwarded to you, please inquire at HRadmin@uab.edu regarding your need to file.

3. I read the "Who Must File" document and I'm not sure if I should file or not. How do I know whether I am required to file or not?

4. Why do I need to file? How does the process work?
In summary, the requirements as they apply to UAB are:
- Persons whose base rate of pay at any time during the last calendar year was $97,000 or more, even if employment during the year was for a period as short as one day
- Persons whose job includes purchasing or procurement authority
- Individuals who served as a supervisor (UAB has chosen to identify these individuals in the HR Admin System as those who served in a job role with a leadership category of manager, functional director or organizational leader during the last calendar year.)
- Executive assistants to leaders in the institution, due to their access to highly confidential information.
Persons whose pay and benefits come primarily from UAB Hospital are exempt from the filing requirement.

5. I think I filed this form already. Can you check to see if I have?
To check filing status, go to ethics.alabama.gov; click on the "Public Records" tab and select "View Filings Here." From the drop-down menu, select "SOEI Forms/Additional Filings" and search for your name.

6. I'm having problems with the electronic form. Who should I contact?
If your web browser is anything other than Internet Explorer 9, please see the Online Form Compatibility Instructions PDF to ensure your computer will work properly with the program. Any problems or errors with the Ethics Commission website should be directed to the Alabama Ethics Commission. 7. I did not receive a confirmation code; did I successfully complete the SEI Form?
As of 2022, tThe Alabama Ethics Commission transitioned to a new system. Users will no longer receive a confirmation code. If you provided an email address during the filing process, you should receive a confirmation email regarding having completed the form. Retain that email for your records.

8. I don't think I should be required to file. What do I need to do?
If you believe you have been misclassified regarding the filing requirements, HR will need a removal request from your supervisor and VP/Dean stating the reason you should be removed from the filing list. Please send all removal requests to HRadmin@uab.edu.
