The UAB Pathology Department provides consultations for clinicians/physicians by our world-recognized pathologists with expertise in surgical pathology and cytopathology. We accept traditional glass slide second opinion consultations or blocks for complex cases.

  • Reports faxed when case is signed out
• Return of slides and paraffin blocks unless permission to retain is granted by consulting physician

Submission Instructions

If you are a pathologist or clinician/physician, please complete the consultation request form, print, and send with the slides or block. To ensure prompt review, all checklist items must be submitted. 

Consultations for patients or clients outside the United States, the International consultation request form should be used.
