• UAB Vehicle Safety Management Program

The Vehicle Safety Management Program ("VSMP") provides overall guidance and explains the responsibilities for departments and drivers associated with operating a vehicle on UAB business. The VSMP also provides guidance related to the use of golf and utility carts at UAB.

• UAB Motor Vehicle Record (MVR) Request and Release

The UAB Motor Vehicle Record (MVR) Request and Release document should be used to request a review of an individual's driving record. MVR reviews are required of all drivers of UAB owned or leased vehicles. Other situations require an MVR review also. Refer to the VSMP or contact Risk Management at 205-934-5633 with questions.

• UAB Vehicle Accident Report Form

The form should be complete by the UAB driver as soon as possible following an accident. This is the driver's opportunity to record the events of the accident in his/her own words.
