Dean's Certification
Many institutions and employers require a student to provide a conduct record check, also known as a Dean’s Certification. Community Standards & Student Accountability is responsible for completing any forms or questions related to a student’s conduct record.
If you have a form provided by the institution/employer, you are more than welcome to submit that to our office for completion. Please be sure to complete any waiver on the form allowing UAB to disclose the information. If the form does not have a waiver or you were not provided a form, please complete the Authorization to Release Conduct Records.
