A physician referral is not needed for an initial appointment, unless required by insurance. To help us serve you as efficiently as possible, we encourage new patients to provide the following information prior to the first appointment: copies of imaging and imaging reports; audiology reports (if applicable); and a medical history. You may also bring these items to the initial appointment.
