Under the direction of Michele Talley, PhD, ACNP-BC, FAANP, FNAP, FAAN, the University of Alabama at Birmingham School of Nursing partners with UAB Medicine to operate the nurse-managed PATH (Providing Access to Healthcare) Clinic in the Community Health Services (CH20) Building at UAB.
This collaborative effort utilizes an interprofessional, team-based model to provide diabetes care at no cost to an uninsured, medically underserved population discharged from UAB Hospital.
Interprofessional Services include visits with Nurse Practitioners, Physicians, Optometrists, Social Workers, Behavioral Health Specialists, Registered Nurses, Dietitians, and Physical Therapists. Assistance in obtaining needed supplies and medications at the lowest possible cost is also provided. Students from a variety of disciplines are integrated in the clinic to learn in an innovative interprofessional model of care.

Diabetes patients being discharged from UAB Hospital: Referrals of patients who are uninsured and have an A1c > 9 can be made using an AdHoc form in the IMPACT system. The care coordinator team will contact the patients who are referred to schedule their first appointment within 2-3 weeks. Hours of Operation: Clinic is open from Monday-Friday from 8:00am to 5:00pm.
Patient appointments are scheduled on: Tuesday & Thursdays from 8:00am to 12:00pm & 1:00pm to 5:00pm
