Form Assembly forms are forms that are connected to Salesforce. Only one form can be added per page in Joomla. If multiple forms are added to the same page, it causes a conflict and the forms will not render correctly. Use cases for these forms include lead forms for capturing prospective student information and newsletter sign-up forms.

How to use
- formid
- The id of form.
- formTitle (optional)
- The title you want to appear above the form.
- titleHeading (optional)
- The heading level that corresponds to where in the html structure this form appears.
- titleClass (optional)
- Add classes to style the title of the form.
- formClass (optional)
- Add classes to style the container of the form.
- source
- The source you want Salesforce to attribute to this form submission. You will need the hidden field ID from Form Assembly and the hard-coded value for the source. An example is tfa_179=Web%20Form%20-%20CSOB
- campaign
- The id of the hidden field in Form Assembly where you want to store the campaign UTM parameter.
- campaignSource
- The id of the hidden field in Form Assembly where you want to store the source UTM parameter. Campaign source is more granular than source above.
- addFields
- Any additional fields you need to prefill in the Form Assembly form for the form to function correctly. Examples include tfa_459=tfa_461 and tfa_366=0124V000001uImQQAU&tfa_46=tfa_365&tfa_55=0014V00001y8MYoQAM&tfa_356=Collat%20School%20of%20Business
