Steps For An Appointment

The Civitan-Sparks Clinics take referrals from family physicians, pediatricians, other health care providers, schools, and family members.

Before appointments can be scheduled, you'll need to fill out our appointment request form. This information is crucial for customizing care with the appropriate clinics and specialists. The form is available online to complete at your convenience.

Referral from provider

Your primary care provider can also start this process by sending an EPSDT referral or the Referral Form below. They can fax referrals and most recent visit notes to us at 205-638-2526. We will have to request a referral from your provider closer to the appointment to bill insurance for services.

Additional Resources

Send us any prior testing that has been completed. If the person who has custody is not the biological parent, please send proof of legal guardianship. We cannot communicate with you about appointments without this document.

Appointment Scheduling and Waitlist

Please be aware that our wait-times are currently 8-12 months for appointments.

Once we receive your completed appointment request form, we'll review the submitted information. Civitan-Sparks Clinics will then contact you to set up your appointment(s). Our team will work with you to find convenient dates and times for your visits.




What to do while you wait

Our current appointment wait-times are 8-12 months, but there are ways you can support your child during this period.

For children under 3 years

  • Early Intervention provides a variety of services in the home free of charge. You can ask for the services without a referral. Call 1-800-543-3098 (TTY: 1-800-499-1816; Español: 1-866-450-2838) to request services.

For children 3-18 years

For children under 8 years

  • Help Me Grow Alabama helps families identify services in their community. They can be reached by calling 833-989-0336 or filling out their online form.

For children with a question of autism

  • The Regional Autism Network (RAN) serves those who have questions, concerns, or resource needs regarding themselves, a family member, friend, client, patient, or student with diagnosed or suspected Autism Spectrum Disorder (ASD).
