For people interested in receiving care at the 1917 Clinic:

- Date of HIV Diagnosis
- List of Current Medications
- Emergency Contact Person and Phone Number
- Referring Physician (if any) and Phone Number
- Type of Health Insurance (if any), Policy and Group Number
- Type of Dental Insurance (if any), Policy and Group Number
- After providing the above information, you will be scheduled to meet with a member of our 1917 Clinic Orientation Team within 5 business days.
For referring clinics or agencies, please complete the "Becoming a New Patient" Form and fax to 205-975-8188. All new patients must complete an orientation visit before their medical provider appointment. This includes people newly diagnosed with HIV, patients transferring care to 1917 Clinic from another facility, and patients returning to 1917 Clinic after > 12 months.
The purpose of the orientation visit is to:

- Collect a complete history of how you are doing (physically, emotionally, spiritually);
- Help you learn about the resources at the clinic and how to access them;
- Obtain initial lab work;
- Schedule your first medical provider appointment.
Once a patient completes the orientation visit, a medical provider is made usually within one month.
For patients with a new diagnosis (no prior HIV care), we offer Fast Track appointments where both the new patient orientation and the provider appointment are on the same day.
