Patient Rights
You have the right to:
- be treated with respect and dignity.
- be informed about your diagnosis, treatment, and prognosis.
- information in your medical record.
- choose advance directives about your health.
- be informed of any research projects that may affect your care.
- have your examination, medical record, and conversation about your health be private and confidential.
- voice concerns about your care and to receive a prompt response.
- translation for speech, hearing, or other important needs.
- ask questions about treatments or procedures which are planned as a part of your care.
- expect a quick response to reports of pain.
Patient Responsibilities
You are responsible:
- for providing information about your health history.
- to follow your plan of care.
- to meet appointments.
- to cancel appointments in a timely manner if you cannot keep your appointment.
- for financial obligations for care and services.
- to be considerate of the rights of other patients and clinic personnel.
- to ask what to expect regarding pain and pain management.
- to ask for pain relief when pain first begins and to assist the providers in assessing your pain.
Cost sharing
- Patients are expected to meet insurance co-pay requirements at the time of service delivery.
- Services are discounted based on a sliding-fee schedule.
- Patients are not denied service based on inability to pay copays, sliding scale fees or other payments.
- Patients who have bills that are in collections are not denied services.
